HEART FAILURE PRIMARY CARE LEADERS NETWORK virtual meeting
In conjuncture with Novartis, the Primary Care Cardiovascular Society are delivering the second Heart Failure Primary Care Leaders Network virtual meeting, to be held online on, Wednesday 2nd December from 6.30pm - 8.30pm.
Heart Failure Primary Care Leaders Network is an opportunity to network and share best practice with like-minded colleagues who are involved in managing heart failure patients in their localities. During this meeting we will discuss what more we can do in primary care to improve heart failure pathways and how we can engage with out heart failure patients, reflecting on discussions from the recent PCCS Annual Conference.
The meeting will be an interactive forum where all participants are encouraged to share challenges and ideas. Dr Matt Fay will discuss the Bradford Heart Failure Pathway as an example of best practice, and other members of the group are invited to share their own pathways as well as any initiatives to improve patient engagement.
