HEART FAILURE PRIMARY CARE LEADERS NETWORK
In conjuncture with Novartis, the Primary Care Cardiovascular Society delivered the second Heart Failure Primary Care Leaders Network virtual meeting, online on Wednesday 2nd December 2020.
Heart Failure Primary Care Leaders Network is an opportunity to network and share best practice with like-minded colleagues who are involved in managing heart failure patients in their localities. During the meeting we discussed what more we can do in primary care to improve heart failure pathways and how we can engage with out heart failure patients, reflecting on discussions from the recent PCCS Annual Conference.
The meeting was an interactive forum where all participants were encouraged to share challenges and ideas. Dr Matt Fay discussed the Bradford Heart Failure Pathway as an example of best practice, and other members of the group were invited to share their own pathways as well as any initiatives to improve patient engagement.
This educational meeting is organised and fully funded by Novartis and is endorsed by the Primary Care Cardiovascular
