There are 6 sections of the programme that will support change within a Heart Failure Service.

The purpose and objective of this programme is:

  1. Prevention
  2. Suspected HF
  3. Optimising Coding Process
  4. Care Management with a plan
  5. Follow up care
  6. Frailty & Palliative care

    7. Within each of the sections there are 3 components of guidance for change:
    • Area of interest: This describes the area of operational service delivery within the section and the reasons for change.
    • Key Actions for change: This describes 3 areas of service delivery that will bring effective quality improvement.
    • Resource: A list of resources (links) to support knowledge and mechanisms for change.
