How will this tool kit help & what is its purpose?

This toolkit will provide a QI framework to all professional practitioners who deliver HF services across primary and community care structures to transform practice and delivery to a gold standard.

The purpose and objective of this programme is to:

• provide information and tools to support colleagues and better understand their own population gaps and care processes.
• understand current systems in place within their own Primary Care Networks (PCN) to improve earlier diagnosis, coding processes and care optimisation (including lifestyle, comorbidity management, medication optimisation).
• support the wider healthcare system to empower patients, avoid admissions, and identify patients who have frailty, are in the last year of life and require a palliative approach.

The scope and core of the programme is to focus on objectives outlined within the NHS Cardiac Pathways Improvement Programme (CPIP) workstream 3.
