Celebrating Outstanding Individual Impact in Cardiovascular Care

This award recognises an exceptional healthcare professional who has demonstrated outstanding commitment, innovation, and leadership in cardiovascular disease (CVD) prevention and management within primary care.

We invite PCCS members to nominate a colleague who exemplifies excellence in cardiovascular care. Whether it’s a GP, nurse, pharmacist, or allied health professional, this award honours those who go above and beyond to improve the lives of people living with or at risk of CVD.

Nominees should demonstrate:

  • A tangible example of excellence in patient care or outcomes.
  • A commitment to quality improvement within their practice or wider system.
  • Evidence of innovation in cardiovascular risk management (CVRM).
  • A proactive approach to collaborative or multidisciplinary working.

Whether it’s a GP, nurse, pharmacist, or allied health professional, this award will honour those who go above and beyond to improve the lives of people living with or at risk of CVD.

Nominations are open to PCCS members and must be submitted by a colleague. Winners will be selected by a PCCS judging panel and announced at the PCCS Spring Conference in May 2026.


HCP of the Year Award – Abstract Submission:

Please structure your nomination of a colleague using the following headings. The word limits are proportional to the judging criteria. The total submission must not exceed 500 words.
  • Project: [300 words] A tangible example of excellence in patient care or outcomes. Include: Evidence of innovation in cardiovascular risk management (CVRM) and a proactive approach to collaborative or multidisciplinary working.
  • Person: [200 words] A commitment to quality improvement within their practice or wider system.
