Recognising Collaborative Excellence in Cardiovascular Care

This award celebrates a multidisciplinary team that has delivered outstanding improvements in cardiovascular care through collaboration, innovation, and patient-centred practice.

Nominees should demonstrate:

  • Implementation of a quality improvement initiative that has enhanced CVRM care.
  • Measurable improvements in patient outcomes or care delivery.
  • Strong teamwork and integration across disciplines.
  • Evidence of innovation, such as new models of care, digital tools, or service redesign.
  • Contributions to practice efficiency or system-wide impact.

This could include GP practices, PCNs, pharmacy teams, nurse-led services, or integrated care teams working together to transform cardiovascular care.

Nominations are open to PCCS members. Winners will be selected by a PCCS judging panel and announced at the PCCS Spring Conference in May 2026.


Cardiovascular Team of the Year Award – Abstract Submission:

Please structure your abstract using the following headings. The word limits are proportional to the judging criteria. The total submission must not exceed 500 words.
  • Quality Improvement Integration: [125 words] Outline how your team has embedded quality improvement into its work. Include methods used (e.g., audits, data analysis, PDSA cycles) and key outcomes.
  • Teamwork & Multidisciplinary Collaboration: [100 words] Describe how your team works across disciplines. Highlight collaboration, communication, and shared goals. 
  • Innovation & Creativity: [100 words] Showcase innovative or creative approaches your team has taken. Include new ideas, technologies, or service models.
  • Patient Impact: [100 words] Explain how your work has made a difference to patients. Include feedback, outcomes, or stories that demonstrate impact.
  • Scalability & Sustainability: [75 words] Describe how your initiative can be sustained or scaled. Consider resources, adaptability, and long-term planning.
