Heart Failure Survey

PCCS & Medtronic – Heart Failure Survey

This survey has been funded and supported by Medtronic.

Thank you for taking the time to complete this survey. The aim is to identify key barriers faced by NHS primary care professionals in diagnosing heart failure and referring patients for device therapy such as implantable cardioverter defibrillators (ICDs) and cardiac resynchronisation therapy (CRT). Your insights will help improve pathways and patient outcomes. 

This should take approximately 5 minutes to complete.
